Hi, my name is Wilmer Rokusek and I am raising funds to help pay for my daughter’s funeral. Here is her story: Jennifer Rokusek was 44 years old when she passed away on May 19,2025 at Ava’s House. Jennifer had Cerebral Palsy. Jennifer was in two different group homes in Minnesota and lastly The Abbott Northwestern Hospital, Minneapolis before we got Jennifer back home to Sioux Falls, South Dakota to Select Specialty Hospital on April 1, 2025. Jennifer’s health continued to decline. Jennifer had a couple Rapid Responses and was transferred to Sanford Hospital. Jennifer was able to go back to Select Specialty. Jennifer was found unresponsive on April 19, 2025. Medical staff issued a code blue. Jennifer was rervived and transferred back to Sanford ICU. Jennifer’s poor body continued to decline as tube feeds had to stop otherwise Jennifer would get aspiration pneumonia and would choke. Jennifer entered Ava’s House on May 12,2025 and passed away on May 19,2025. Jennifer had spunk and determination that we could all learn from. We are asking for help to pay for funeral expenses and numerous trips to Minnesota and back.
